Protected Health Information (HIPAA)
As a licensed home health provider, Havens Corner Home Health Care, LLC is a HIPAA-covered entity. PHI is not collected through this public website; if you share health details with us here, please share only what is necessary to route your inquiry.

SMS / text message terms
Program description. Havens Corner Home Health Care, LLC sends text messages related to your inquiry, scheduling, care coordination, employment application status, document reminders, and general account or service notices. Message frequency varies based on your interaction with us.
How you opt in. You opt in to receive text messages from us by:
- Providing your mobile number and checking the SMS acknowledgment box on our contact or intake forms;
- Providing your mobile number to a staff member and agreeing to be contacted by text; or
- Texting us first from your mobile number.
Your consent to receive text messages is not a condition of purchasing any product or receiving any service from Havens Corner Home Health Care, LLC. You can request the same information by phone or email instead.
How to opt out. Reply STOP to any text message from us at any time to opt out. You will receive a single confirmation message and will not receive further texts from that program. Reply HELP for help, or contact us at 614-655-8309 or office@hchomehealthcare.com.
Message and data rates. Message and data rates may apply depending on your mobile plan. Contact your wireless carrier for details. Message frequency varies.
Supported carriers. Messaging is supported on major U.S. carriers. Carriers are not liable for delayed or undelivered messages.
Privacy of SMS content. We do not share mobile numbers or the contents of SMS conversations with third parties or affiliates for their marketing purposes. Mobile information is shared only with vendors that help us deliver messages, under contract, and as otherwise described in this policy.
PHI over SMS. Standard text messaging is not a secure channel. Please do not send Protected Health Information over SMS. For clinical information, we will direct you to a secure channel (phone call, secure portal, or in-person conversation).
Data security
We use administrative, physical, and technical safeguards designed to protect the information we hold, including access controls, encryption in transit, and audit logging within systems that handle PHI. No method of transmission or storage is 100% secure; we do not guarantee absolute security.
Retention & deletion
We retain information for as long as needed to provide services, meet our legal, licensing, and accounting obligations, and resolve disputes. Clinical records are retained for the period required by Ohio law and applicable federal rules. When information is no longer needed, we dispose of it using secure methods.
Your rights & choices
- Access, correction, and deletion. You may request access to or correction of the personal information we hold about you by contacting us. Clinical record requests follow the process described in our HIPAA Notice of Privacy Practices.
- Marketing communications. You can opt out of marketing emails using the unsubscribe link in the email, and opt out of SMS by replying STOP.
- Cookies. You can control cookies through your browser settings. Disabling cookies may affect site functionality.
Children's privacy
Our website is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through this site. If you believe a child has provided us information through the website, contact us and we will delete it.
Changes to this policy
We may update this policy from time to time. When we do, we will update the "Last updated" date at the top. Material changes will be communicated through the website or, where appropriate, directly to affected individuals.
